RELEASE
Definitions:
“I” – the party contracting with Pet Taxxi and/or Nashville K-9 for services to be rendered.
“Company” – Pet Taxxi and Nashville K-9, any and all subsidiaries, any and all of their employees, servants, agents, successors, heirs, executors, officers, directors, shareholders, and administrators
“Pet” – the animal provided by or on behalf of its owner to Pet Taxxi and/or Nashville K-9 for services to be rendered.
“Transport” – any and all actions taken by Pet Taxxi and/or Nashville K-9 including, but not limited to, the handling, care, transportation, boarding, training, and/or otherwise services contemplated by the parties hereto.
I understand and agree that Pet Taxxi will transport my pet with the period of time I have chosen. I agree to indemnify and hold harmless the Company for any actions or costs incurred related to the transport or future transports of my pet. I agree the Company is not responsible for any injury and/or damages to my pet or any persons incurred during or associated with transport, handling, or any other action or inaction related to my pet.
I understand that my pet will be photographed and video will be taken of him/her. I allow Pet Taxxi or Nashville K-9 to use these photographs or video for the promotional and/or advertising purposes for Pet Taxxi Nashville K-9.
I understand that I will be responsible for any veterinary or other costs that may be incurred as a result of my pet’s transport including, but not limited to, injury or sickness to the animal and/or persons. I understand that the Company will not be responsible for injury or the death of my pet and, in case of emergency, a veterinarian will be sought for treatment and if that veterinarian is able to treat my pet, I agree that I will be responsible for all costs incurred as a result of treatment by said veterinarian consistent with my desire for the well-being of my pet.
This agreement shall be binding upon the heirs and assignees of the parties and shall be governed by the laws of the State of Tennessee.
I agree to indemnify and save harmless the Company from all further claims or demands, liens, cost or expense arising out of the injuries or damage sustained by me and/or my pet. It is expressly warranted that I have paid or will be solely responsible for the payment of any and all past, present and future expenses, including expenses for medical services, liens, doctor’s services, drugs and medicine, arising out of the performance of the services contemplated herein.
I affirmatively represent that no promise, inducement or agreement not contained herein has been made to me, and that this Release and its terms are contractual and not a mere recital.
